Activation of the Brain Cortex Measured by Near-infrared Spectroscopy during Ego State Therapy with EMDR in Dissociative Disorder

Using ego state therapy with eye movement desensitization and reprocessing(EMDR) is one of the treatment protocols for dissociative disorder. However, the mechanism of action in the brain is not yet fully discovered. Therefore, the study was designed to examine relative changes in [oxy-Hb] in the brain during a session of EMDR with ego state therapy by using near-infrared spectroscopy(NIRS).
There was a sudden increase in the activation of the right superior temporal sulcus during eye movement. As ego state therapy was introduced, the blood flow decreased. With a specific ego state recalling unpleasant memory, the heart rate accelerated and the brain blood flow increased in the area of prefrontal cortex and temporal gyrus, whereas EMDR was applied to process the unpleasant memory, the blood flow generally decreased, except for the area of prefrontal cortex.
These results suggest, from the neurobiological studiesʼ perspective, that ego state therapy combined with EMDR appears to be effective in inhibiting the excessive activation of the region adjacent to right superior temporal sulcus in cases of dissociation, and thereby allows processing unpleasant memory which is stored in specific parts.

さらに司法機関において,どのような条件下でEMDRの適用が可能かを考察した。研究対象は,犯罪を起こして逮捕され家庭裁判所で審理中の非行少年(14歳〜19歳,男女)であり,有効性・安全性・妥当性の3つの視点に基づいて,3事例が選ばれた。
測定の結果,3事例のうちの2事例で,1回の介入による明らかな改善があった。しかし1事例では1回の介入による改善が見られず,ケアが不十分である疑いが示された。非行少年に対して策定したガイドラインに従うことで,EMDRが症状や問題性の改善に有効である可能性が示された。

The Possibility of EMDR Use to Juvenile Delinquents
─A Short-term Intervention in the Inquiry Stage at a Family Court─

The purpose of this study is to examine the effects of the EMDR use to juvenile delinquents, such as improvements in the area of maladaptive physical and mental symptoms, emotional control, self-recognition and attitudes toward others. It also examines under which condition in the judicial proceedings, it is possible to apply the EMDR method to juveniles. Three delinquent cases which were on trial in the Family Court, were selected as participants from the view points of effectiveness, safety and validity. Results showed clear improvements by one EMDR intervention in two cases out of three. Insufficient care could be the reason for the absence of improvement with the third participant. The results of this study suggest that the EMDR is effective for the improvements of the various symptoms and problems of the juvenile delinquents, if it is properly applied under the certain guideline set for them. 本論では,妹への暴力を主訴として相談に訪れた小4男児と家族への心理治療の過程を時系列に記述した。妹への暴力は,母が本児を愛せないことから生じていた。親子の関係性における悪循環を改善するためには,母のトラウマに対してEMDRを行うことが有効であった。本事例の治療経過を通して,EMDRを効果的に利用するためには,事例全体をシステミックに見立てることが重要であることを示した。

Psychotherapy for Sibling Abuse with a 4th Grade Boy and His Mother

Traumatic experiences which should be targeted in EMDR session are not specified in many of cases when parents come in for psychotherapy to improve their children’s problems. However child’s problem is an SOS signal from the family system which suffers from complexly intertwined big and/or small traumatic memories.
The process of the psychotherapy was reported in chronological order in this paper. Because his mother could not love him, the sibling abuse by the 4th grade boy was happening. EMDR sessions for his mother’s traumatic memories were effective to improve a vicious circle in the mother-son system.
Through the therapeutic process of this case, the importance of systemic case conceptualization to apply EMDR effectively was indicated.Japanese Journal of EMDR Research & Practice, 4 ; 41-51, 2012
